AAV email improvements/fixes

Many of you have been experiencing problems with AAV emails on your FREPs. I think I have found and fixed one of the problems. I've also added a lot of logging to help with future debugging, so if you have email problems going forward, please let me know.

As before, when reporting missing email, please include this information:
date
time
reason (FREP submit, FSACARS submit, FREP edit, FREQ submit, FREQ done)

Submitted FREPs were all received ok, but I did not receive any e-mails for the FREPs being accepted and I did not receive the "FREQ completed" one either. I don't know the exact time they would have been sent, but the FREPs were accepted and the FREQ completed sometime between 1300z yesterday (11th Feb) and now.

I'm going to have to add some more logging to track this down.
