Sounds to me like you have two different problems. I doubt the CIVA INS is related to the engine failure. I suspect the engine failure is due to empty fuel tanks, fuel pumps turned off, or not getting power to the fuel pumps because you're still running off battery. It would help to diagnose your problem if you posted screenshtos of the electrical and fuel panels before or during the failure.
Now, on to the CIVA INS. Do you have the CIVA INS files installed in a sub-directory of your FS9 folder? Does the INS show up in the panel? Are you using version 1.4 or 1.5? You want 1.5, it has some bug fixes and allows you to save your CIVA INS state when you save a flight.

Not sure exactly what sequence of events got you to this point. I'll point out some things and maybe we can work out what's happening.
Power looks good. You're running off APU.
On the fuel panel, your fuel flow switches are off. One thing it took me a long time to realize is that these fuel flow switches are connected to the fuel flow switches on the throttle quadrant below the throttles. Did you turn these on when starting the engines? Did you turn them on here, or on the throttle quadrant. Note that if you try to turn them on in BOTH places, you will turn them on when you hit the switch in the first location, and turn them off when you hit the switch in the second location. I never touch these fuel flow swithces on the fuel panel and only use the ones on the throttle quadrant.
