John,
After you submit a FREP (manually or with FSACARS/SIMACARS) it waits on our website until your hub manager approves or denies the FREP. During this time, you can see them by clicking on the "Edit a FREP" button under our Resources menu. Once the FREP is approved, it will show in your logbook. When the FREP is approved or denied, you should receive an automated email from the website.
Looking at your logbook and your waiting FREPs, I see your most recent SIMACARS FREP was for flight SKW5551 on Aug 29, and you have NO FREPs waiting.
Did you receive any emails about denied FREPs?
I'll go through the website logs to see if I can identify any issues with your SIMACARS client FREP submissions.

Hi John,
You have submitted a number of FREP's without any aircraft type. I have denied these and asked you to refile them with an aircraft type (Which you did). The last 3 FREP's you filed did not have aircraft type in them and these were denied with a note asking for aircraft type.
I believe you get notifications by email when the FREP is processed and if it is Approved or Denied.
I did send you an email to which I have had no reply.
Please let is know if you are not receiving your emails and we will look in to it.

In the attachment you posted here you can see where the aircraft type is missing, you can check and edit the FREP after you post it if required. Notice also that somehow it also created a duplicate FREP. I am not in a position to diagnose any faults with your system, all I can do is work with your posted FREP.
By the way, have you been receiving the emails after your FREP has been Accepted or denied?

I have an idea on the missing aircraft equipment. On your SIM ACARS Logbook page, click on the little blue letter i in the blue circle for one of your flights where the equipment was missing from the FREP. Check the Aircraft and Aircraft Type ICAO fields. The Aircraft Type ICAO field is used to populate the aircraft equipment field in your FREP. If by chance you are only putting the aircraft type in the Aircraft field, that would be why your FREPs were missing the aircraft equipment type.
The SIMACARS usage instructions post is not clear on this, so I have just added a step to clarify which field should be used.
